Studying the wonderful GIF it's not clear to me the sequence of the two blows.

There is blood on the nipple ( close up ), then a brutal stroke hits the nipple wiping out the blood (?), then a second blow ( not close up frame ) hits again the poor nipple with more blood.

Maybe the stroke is just one by two differenti point of views ( cameras ) ?
