Ah yes the loincloth / panties thing of otherwise great 70's sexploitation movies ruined a lot of them for me, in situations where any reasonable GIMP would expect to be fully naked. Alas they kind of had to do that in order to get a normal theatrical release, without which you could not monetize a fully funded Hollywood film before the introduction of home video.

Incidentally, regarding the lack of bent arms in this particular scene, I’d like to think that maybe there is some kind of hidden mechanism which jabs spikes into her spine. Regardless, she still sells the agony exquisitely.
