I took a closer look look at the pin & screw, and I now think it's a bit faked. It looks to me like the screw was run through the closed jaws of the pin, then cut between the jaws. The two halves of the screw aren't quite in a straight line like they would be if it was in one piece. So it's a bit of an illusion.
Having said that, and my apologies to Ms Dede, it is a very good idea for a nice nipple torture if done for real.

I tend to be a problem solver, and I'm always looking to improve something. I have given the screw-clothespin thing some though, and have and idea how to make it a rather nasty little device. I'll try to keep it simple, so bear with me.........
1. Select a drill bit slightly smaller than the screw you're going to use, and drill a pilot hole through the end of the clothespin. The hole should be large enough to accept the screw without splitting, but a fraction smaller so the screws threads will bite.
2. After drilling the pilot hole, select a slightly larger bit than the size of the screws threads. Use that to drill one of the holes on one side of the clothespin, but not the other.
3. Now thread the screw through the holes to pre thread the smaller one. Be sure to use a standard sheet metal type screw, and not a wood screw.
4. Remove the screw, put the clothespin on your volunteer's nipple, cuntlip, earlobe, etc., and press the screw into the larger hole. With a screw driver, drive screw into the flesh till it catches the threads in the other half of the clothespin.
5. Keep tightening the screw, and the head will begin to press the jaws of the clothespin together, crushing the screwed nipple in the process.
6. Don't forget to gag your volunteer, cause it's going to hurt.

I don't recall seeing this device, but it looks menacing and is relatively cheap. I think it will also be at hand in many a home and office. It would work particularly well on fingers and nails.

Ah yes... the lowly staple puller. A much overlooked little device that can really put tears in a girls eyes.
Simple to use:
1. Grip, squeeze, and embed into breast or any available fleshy area.

2. Apply a tight wrapping of these around the closed puller (see below).

3. Enjoy.
