I got the add the genius of iron maiden is cone shape spikes acted like a compression bandage in that the damsel could last days but once the door is open, she bleeds out immediately, so once she goes in, there is no hope of coming out alive.
You could say it both torture and executional device. Very slowly tortured to death.

And if proper thought was given the length of the spikes (in certain ares where major organs are), death might be put off for a very long time if given nutrients and hydration.

Also think of the body smell after a couple days. Urine and possible shit from Toxic Hell double bean burrito she ate because whatever goes in the stomach must come out for sure. Then followed by flies. Ugh! Will Burner

That's only for bargain-basement Iron Maidens, Will. The better models come with internal water jets and drains to wash that disgusting stuff down into the sewer system and keep the maiden in the Maiden smelling nice and fresh!

Ha ha! That's why I think all those "dungeon" scenes with skeletons in chains... improbable. Unless use of the chambers was very infrequent.

Or ... giant mutant rats with a taste for human flesh?!
