Just fantasie and not possible in real movie productions, but a rough idea how a movie like this could look:

There is a practical reason you won't find much of the kind of content you're looking for and it's not because no studio wants to make it. Both the gentleman's agreement which ruled the print and film industries before the Internet came along and the credit provider's agreement hammered out by Peter Ackworth and Kink.com after Insex and some other studios were shut down specifically forbid PIV ("Penis In Vagina") sex in a BDSM context, presumably because it looks too much like real rape. Any studio that wants to provide this will have to resort to sketchy credit processors that are prone to go away suddenly or "creative" means of payment like bitcoin and gift cards.

In my opinion in hard bondage is no room for conventional sex, no matter the position or intensity. Penetration is generally speaking acceptable in those situations but it can't be a reward for a girl, it must be just an add on to the humiliation process. It's important to note that this embargo isn't about anything legal. Insex was shut down because DOJ sent national security letters to their bankers accusing them of funding terrorism and demanding that they stop doing business. This, not what Insex was doing, is an illegal as fuck abuse of the Patriot Act but who's going to stop them? Everyone found out what was going on because one of Insex's processors showed Brent Scott (aka PD) the letter, even though that was technically illegal.

Ackworth saw the writing on the wall and went to his credit people to ask what they would need to stand up to this and push back. The new agreement is the result, and just about everyone who accepts credit cards abides by it. It is very long and specific and changes occasionally. It requires a pre-scene interview, a post-scene interview, and imposes a lot of other restrictions (for example, the scene must stop immediately if the model starts to cry). Many of the previously independent content creators, including the Insex crew, went to work for Kink rather than deal with the whole mess themselves. This means they now have to follow the rules.

In the pre-Internet days the rules were established by a loose coalition of publishers who wanted to avoid having their wares seized by podunk sheriffs in weird venues. Everyone with a practical distribution network basically agreed not to compete with each other on this kind of content, and so if you were a creator and made a film or photo shoot like that nobody would publish it and you probably couldn't publish it yourself because you didn't have the distribution network. Some material was available out-of-band through mail order and the old time under the counter channels, but that was unusual and hard to hook up with.

It was Brent Scott realizing that the Internet bypassed the distribution problem that put all those people out of business and gave us seven years or so of pretty much anything goes internet content. Now the bottleneck is payment, and it is only producers willing to deal with sketchy credit processors (which are prone to disappear overnight keeping a lot of your money), or use unusual methods like crypto or gift cards that limit their potential audience to people willing to resort to such things. This means you might have a few edge players willing to skirt the rules but for the most part professional content is going to follow them.
