From all I've read about torture - a lot over decades - real torture sessions were grimy and brutal with fists and domination rape being the primary tools. Medieval torture chambers were ghastly, dank, filthy, the stench of excrement overwhelming. As mentioned, most religion-based tortures did not feature nudity. I think we all missed the O.P's question and intentions.
To be clear: there's nothing in common between the hardest BDSM productions and REAL torture, the former is consensual and safe, (even if the pain is real), the latter is a potentially lethal criminal action.
Whatever cruel and painful the video productions of Pedro, Belle, Lupus, etc... could be, they can NOT be considered as "torture" in any ways, not even a credible simulation. The purpose of "real" torture is to extract some hidden information(s) from the tortured victim, not to satisfy some sexual fantasies. Perhaps I should clarify. I feel confident in saying that no one here advocates real torture, 100% non-consensual, flesh-shredding, capable of killing TORTURE. I can only try to express how my fantasy plays out for me.

It's an odd dichotomy and involved. When I watched classics like Poor Cecily and Ilsa, I willingly/eagerly suspended my disbelief for better fantasizing. I have enjoyed several Mood and Elite Pain vids because they have whiplash simulation down, apparent savagery and accurate sound effects.
