I can't help with the id, but I can tell you it's old. I've had them since around '02 or so, so they are pushing 20 at best.

Some wannabe PD makes a not so healthy looking girl straddle an electric "pony". On the plus side she seems to be in a lot of genuine pain and screams nicely. First she tastes a few whips (and electricity) then gets prodded with an electrified metal rod. Not great, not bad. Couldn't find the video in HQ, sorry.

That was one of my earliest exposures to electro on a forum. It's always been one of my favorites ever since. It does look like a video but I have no idea the source nor the model.
